  • It seems like the interviewer kind of wants to avoid the ketogenic diet and just call it or just talk about a “low carb” diet but dr. Volek is making it clear that if you’re going to do a low carb diet you have to increase your level of fat consumption if you want to have energy because a high protein diet with low carbs isn’t healthy. Nor will you have much energy.

  • Carbohydrate Intolerance And Insulin Resistance

    Type 2 diabetes is a disease of high blood sugar. It can also be thought of as carbohydrate intolerance or insulin resistance. That means when someone with type 2 diabetes eats carbohydrates, it causes their blood sugar to rise above a healthy level.

    Everyone has a different carbohydrate tolerance. One person may be able to eat a carb-heavy diet with no problem, and another may get blood sugar spikes and gain weight from eating very few carbohydrates. Both people can be healthy, as long as they are eating within their personal tolerance levels.

    How Type 2 Diabetes Became An Epidemic

    52 percent of adults in the United States have type 2 diabetes or prediabetes, which means that you are more likely than not to have diabetes or be developing it. How did we get here?

    Well, in 1977, the U.S. government recommended new dietary guidelines. Remember the food pyramid?

    The food pyramid recommended 6-11 servings of carbs per day, and very little fatâa low-fat, high-carb diet. As we outlined in our last video, type 2 diabetes is a disease of carbohydrate intolerance. Someone with type 2 diabetes or prediabetes has a low carbohydrate tolerance, so eating carbs will lead to exaggerated blood sugar spikes. While those with a high carb tolerance may be able to eat a carb-heavy diet and remain healthy, someone with a low carb tolerance will experience chronic high blood sugar and likely even weight gain if they eat a high-carb diet.

    Soon after these guidelines were recommended in 1977, type 2 diabetes prevalence increased dramatically, and it hasnât slowed down since.

    There are cultures who have thrived for centuries on high-fat, low-carb diets, such as the Masai warriors and Inuits.

    Ketogenic diets have been used as medical treatments for a long timeâspecifically, to treat children with epilepsy. In the past 20 years, many endurance athletes have started adopting low carb and ketogenic diets for improved performance.

    The Advice Wasnt Working

    She worked as a primary care physician for eight years. It was in the daily treatment of patients with diabetes, obesity and chronic disease that she began to feel that something was not right in the way doctors had been trained to handle these conditions. The advice and prescriptions she was doling out to patients did not make them better. It is so demoralizing to see patients coming back worse and worse.

    Some doctors blame the patient for not being compliant, but not Sarah. She knew they were following her advice. I had the first hint of it as an exercise physiologist. I had worked counselling obese patients. I realized at that time some of these patients were exercising all the time, seemingly doing everything right, and still they were struggling with weight.

    Sarah, 46, now laughs in disbelief when she describes how for many years she truly believed in the Dean Ornish school of health advice, promoting a low-fat, plant-based diet with plenty of whole grains, fruits and vegetables and lots of exercise. I preached the low-fat-eat-less-exercise-more dogma for years. That is what we were taught in medical school and that is what I told patients. But it didnt work. Patients never got better.

    I spent a year getting ready . I got board certified in obesity medicine and read every research study I could get my hands on. And it became exceedingly evident when I read the literature that low fat had nothing to support it.

    Research On Ketogenic Interventions For Type 2 Diabetes

    Dr. Sarah Hallberg: What happens if I follow a low carb/ketogenic diet and start eating carbs again?

    Clinical trials have proven a low-carb, high fat diet to be significantly more effective than programs that encourage you to eat less and exercise.

    In our clinical trial, Virta patients lost almost 12% of their starting body weight in 6 months¹âthat’s nearly 3x the weight loss of commercially available weight loss programs.

The medication approach focuses on management of diabetes, not reversal.     What Is A Carbohydrate

    Itâs one of the main energy sources in many foods, including bread, pasta, fruit, and starches. When carbohydrates are digested, they are broken down into glucose in the blood. You may know this as blood sugar.

    The body allows for only 1 teaspoon of sugar per 5 liters of blood, but there are 9 teaspoons in a cup of cooked rice.

    The excess glucose in your blood is pulled out by the hormone insulin. In a person with a high carbohydrate tolerance, this process works well and excess blood sugar is promptly removed.

    In a person with carbohydrate intolerance, type 2 diabetes, or prediabetes, this system breaks down. The body loses its insulin sensitivity and more and more insulin is required to remove the excess blood sugar.     LOS ANGELES Carbohydrate restriction is a viable patient choice for type 2 diabetes reversal, according to Sarah Hallberg, DO.

    “Nutritional ketosis supports diabetes reversal by reducing insulin resistance while providing an alternative fuel to glucose with favorable signaling properties,” she said at the World Congress on Insulin Resistance, Diabetes, and Cardiovascular Disease.

    Low-carbohydrate nutritional patterns including ketosis have extensive clinical trial evidence for improvement of type 2 diabetes, including preliminary results from a 5-year study of 465 patients enrolled in the Indiana Type 2 Diabetes Reversal Trial that Dr. Hallberg is overseeing in her role as medical director and founder of the medically supervised weight-loss program at Indiana University Health Arnett, Lafayette.

    “The ketogenic diet is not a fad diet, it’s what we used to treat people with before the advent of insulin,” said Dr. Hallberg, who has been recommending and counseling patients with type 2 diabetes to follow a ketogenic diet for nearly 10 years. “Of course, insulin has been wonderful. It’s saved so many people with type 1 diabetes. But we also misused it in type 2 diabetes. Instead of counseling people the way we used to about the food that they’re taking in to control their blood sugar, we’ve just been putting on medication, including insulin.”
